Self Pay Information
If you are uninsured or have commercial insurance, you can choose to self pay at Endo For Kids.
If you have commercial insurance, you will just need to complete an Insurance Opt-out form in our office. You may opt back in to using your insurance by requesting and completing the opt-in form when you check in for your office appointment.
If you are uninsured, you may also request a Good Faith Estimate by calling our office at 865-546-3998.
